Up to 270 women in England may have died because they were not notified of their final routine screening for breast cancer.

UK Health Minister Jeremy Hunt told parliament that, since 2009, 450,000 women aged 68-71 failed to get invitations.

Hunt has announced an independent review and apologised to the women and their families.

He described oversight of the screening programme as "not good enough".
