The Blue’s appeared to be heading for the full three points as the game went to stoppage time.
But Van Persie thundered home a rebound on 93 minutes after a powerful Marouane Fellaini header was saved by Chelsea goalkeeper Thibaut Courtois.
The late strike sent the Theatre of Dreams into wild celebrations and Mourinho and his former Barcelona boss Van Gaal home with one point each.
